Placement Co-ordination Team
Placement Co-ordination team/Service brings together all placement requests either from Family Support Panel or as a result of an emergency.
The Placement Co-ordination Team consists of two Placement Co-ordinators and two Administrators responsible for co-ordinating Placement Co-ordination and Multi Agency Placement Team information.
Referrals are discussed weekly at the Placement Co-ordination Meeting where there is representation from Fostering, Residential and Care Management.
The Placement Co-ordination Service liaises with Area Teams and Resources collecting information and progressing appropriate referrals.
The Multi Agency Placement Team Administrator collates all information for the Multi Agency Placement Meetings, which are held monthly and will seek out appropriate external placements when funding has been agreed.
The Placement Co-ordination Service provides information and data on all internal and external placements to inform service planning.
