6.16 Protocol Regarding Health Assessments - Who Does What |
- The Looked after children health team will undertake assessments of LAC living in Bradford and will liaise with the local health team re children living outside Bradford;
- When children become looked after there will be an assessment with the doctor – form IHC will be completed (age appropriate forms);
- Reviews will be undertaken by the LAC nurse – form RHA;
- LAC health team will also send out SDQ forms to carers for completion and collate these forms;
- LAC nurse will make an appointment for the health assessment /review and send this to the carer, and email allocated social worker to let them know when this will be;
- Doctor/ nurse will see the child and complete the assessment form;
- Doctor/ nurse will write up the assessment and complete the health summary report and health recommendations;
- If the child /young person fails to undertake a health assessment health team will contact social worker regarding helping this to happen;
- The LAC health team will put information on health records;
- LAC Health team admin will input ICS – update immunisations, record dental visits, record carers SDQ report results, put date of health assessment in box;
- Health team will then email the health summary and recommendations to the allocated social worker/case worker;
- The social worker/case worker must immediately;
- Read the report, and take necessary action (e.g. discuss with carers any issues from the plan);
- Send a copy of the summary to the carer;
- Upload the report to CIVICA under C11;
- Check that the date of the assessment has been entered under the Treatments tab on LL and add in the description box see CIVICA C11 YYYYMMDD Health Assessment;
- Ensure that the health dimension of the Care Plan is updated to reflect the health Assessment at the next stage of the Planning /review cycle.
- If the child has an adoption medical or other significant medical examination, it will be useful for the worker to take copies of recent medical assessments undertaken by LAC health team. Contact the LAC health team if you do not have a copy;
- A copy of the LAC health summary should be taken, by the allocated worker, to the LAC review for the IRO;
- SDQ scores – if the score is above 14 discussion should take place between the allocated worker, the LAC nurse and carer to see if any further action is needed regarding meeting the child/young persons emotional well being needs.
Other Actions:
- New social workers/ CRW’s/ residential staff working with LAC to be encouraged to visit LAC health team as part of their induction;
- Training /briefing required for CSC staff regarding SDQ’s.

Part C should be retained in the young person’s health record and a copy sent to the social worker. It is good practice, with appropriate consent, to share this information with the young person’s current and future carers. This summary should also be shared with adoption and fostering panels. For adoption only, a copy of this entire form will be sent to the young person’s adoption agency.
